New patients
Welcome! Here’s your first visit.
We ask all new patients to fill out the health history form before their first visit. Here it is, with what to bring and what happens when you come in.
Before you come
The new patient forms
Download the forms, print them, fill them in at home and bring them with you. They cover your contact and insurance details, the office’s financial policy, your complaint (what hurts, how much, and what makes it better or worse), your health history, and the informed consent.

What happens
History, exam, then a plan
Your history
Your history tells us where to go with your treatment: past exams and diagnoses, X-rays, MRIs or CT scans, medicines, allergies, surgeries, and your daily life (work, activity, sleep and eating habits).
The exam
A respectful, noninvasive exam that may include flexibility, posture, strength, balance and reflex testing, and palpation to find tender or tight areas. More tests only if your case calls for them.
Your plan
A consultation about your goals and needs: a plan for treatment, home exercises, and, where it makes sense, a referral to another specialist.
Insurance & payment
The money part, plainly
- We file insurance claims as a courtesy and will help you receive your maximum benefits.
- Your co-payment is due at the time of service.
- We take cash, checks, Visa, Mastercard and Discover, and can set up automatic monthly billing to a card.
- Deductibles, co-payments and anything your plan doesn’t cover are the patient’s responsibility.
- Senior citizens on Medicare are welcome.
